1) Gain an understanding of our client’s daily routine and allow them to retain a sense of control as we work together to keep them safe. This can include assistance with Activities of Daily Living, Medication Reminders, Grooming and Hygiene, help running errands, and Transportation to and from appointments.
2) Conduct a home safety evaluation and incorporate specific safeguards outlined in our Fall Prevention program.
3) Incorporate techniques of cognitive stimulation by getting to know our client’s life story, understanding their passions and interests, validating their thoughts and feelings, and participating in their favorite games or hobbies – all of which can promote a positive bond and improve mental health.
4) Provide timely communications to keep the concerned family up to date on the condition of their loved one. We are the ongoing eyes and ears in the home to monitor any change in behavior, provide positive reinforcement, notify our RN if the client needs to be re-assessed or the care plan needs altering, and to consistently take proactive steps to keep the client safe at home.
Nearly 20% of seniors experience rehospitalizations within 30 days after their initial discharge from the hospital. While most patients are eager to leave the hospital and get back to the comfortable, familiar confines of their own home, they often underestimate their change in condition and the impact it has on their Activities of Daily Living. Rise Up Homecare’s Transitional Care program provides services such as Ambulation assistance, Transfer assistance, Conducting a home safety inspection, Bathing assistance, picking up groceries and prescription medications, Medication reminders, Transportation to and from doctors appointments, and much more.
